As an out of network provider, if needed we will provide you with an itemized receipt with diagnosis codes after your visit. You can submit this bill to your health insurance as an out of network benefit.

Depending on your insurance plan benefits, doctor-supervised weight loss services may be covered. Our services may be covered by your flexible or health spending account (FSA/HSA).

Please check coverage with your insurance and FSA/HSA plan prior to your appointment. Patients are responsible for payment of all charges at the time of appointment.

For your convenience, we accept cash, Visa, MasterCard, Discover, American Express, and CareCredit.

If Dr. Richardson suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require three business days for the processing of routine referrals.

Family Weight & Wellness Clinic and Medi-Spa offers convenient financial payment options using CareCredit, a healthcare credit card for your deductible or to pay for treatments and procedures.

Weight Loss & Hormone Therapy Patients
In order to allow us to provide the best and most convenient services for all our patients, it is important that we have a No show /Cancellation policy in place. This will cover part of the business costs, and also will give other patients an opportunity to be seen earlier if possible.

Therefore, we require at least 24 business hours’ notice prior to your scheduled appointment if you are unable to keep it as scheduled. If you do not call to cancel your appointment within that 24-hour window, there will be a $50 fee charged to your credit card at the end of that business day.

Medi-Spa Patients
If you are unable to keep an appointment, you must notify Family Weight & Wellness Clinic and Medi-Spa at least 48 hours before your medi-spa appointment time. If for any reason you do not cancel or reschedule an appointment at least 48 hours prior to the cosmetic appointment, you will be charged a $50 fee to your credit card on file. The no-show fees are part of your account balance and must be paid BEFORE your next appointment can be scheduled.
